Commuting issues

I was reading a document (not sure where) about the ammount of bicycle commuters have "incidents" with vehicles etc on Sydney streets and it said that many were unreported.
I know alot on this forum commute to work and i expect that many people have as many near misses with vehicles as i do.

So perhaps others would like to create a record whenever they have a near miss, hit or something whilst on their bikes (scumming, mtb - whatever)
This "record" could later be used for lobbying etc (maybe - I'm not sure)
A little detail perhaps would help.

I'll start off:-

This morning (Wednesday 18 March @ 7:30am)
Travelling north up Ben Boyd rd towards Military Rd Neutral Bay, a dark coloured Toyota Camry glanced me twice in the space of 100m
I was travelling on the white line to avoid sationary cars openning their doors and in his hurry to pass his side mirror hit my elbow.
As he has to stop for a light at Yo street i passed him on the inside lane (there were no cars parked there then) while he had to wait for a car turning right.
Soon after he shot past again swerving into the right side of Ben Boyd rd as i had moved into the centre of the road to turn right onto Military Road.
I was signalling at the time and my arm was hit as he went past. I raced after him to yell at him and found him on the phone.
Luckly i was riding my mountain bike at the time as my balance ins't nearly as good on my racer.
